    Dislocation of the ankle without associated fracture or wound is an extremely rare injury. [1] Fahey and Murphy [2] classified tibio-talar dislocations into anterior, posterior, medial, lateral, superior or combinations of these basic displacements. Most of these are either open and/or with an associated fracture of the tibia, fibula or the talus itself. Of these the posterior-medial dislocation has been described most often in the literature. [1],[2],[3],[4],[5] Most authors have described this injury in young adult males. Falls, road traffic accidents and sports have been described as the most frequent causes of these injuries. Forced inversion or eversion with axial loading in a maximally planter-flexed foot is thought to be the cause of this injury. The patho-Anatomy of this injury has been dependent on findings during surgical repairs and has not been described accurately. We are reporting a case of closed posterior dislocation of the ankle without fracture in an 18-year-old male patient following road traffic accident. The most probable mechanism is forced forward displacement of the tibia leaving the talus behind. The patho-Anatomy as evident from the magnetic resonance finding is also being described. The injury is being described not only for its rarity but also to discuss its unique patho-mechanics, mechanism of trauma and its prevention.

    PATIENTS AND METHODS: We prospectively evaluated 48 patients with a clinical suspicion of thoracic outlet syndrome. Standardized provocative tests, an electromyogram and somatosensory evoked responses, a Doppler ultrasonogram, and a helical CT arterial and/or venous angiogram with dynamic maneuvers were done on each patient. The final diagnosis was established by excluding all other causes based on all available data. The agreement between the results of each investigation and the final diagnosis was evaluated. RESULTS: Provocative tests had mean sensitivity and specificity values of 72% and 53%, respectively, with better values for the Adson test (positive predictive value [PPV], 85%), the hyperabduction test (PPV, 92%), and the Wright test. Using several tests in combination improved specificity. Doppler ultrasonography visualized vascular parietal abnormalities and confirmed the diagnosis in patients with at least five positive provocative tests. Electrophysiological studies were useful mainly for the differential diagnosis or for detecting concomitant abnormalities. Although helical CT angiography provided accurate information on the location and mechanism of vascular compression, the usefulness of this investigation for establishing the diagnosis of TOS and for obtaining pretherapeutic information remains unclear.

    Disability associated with mental disorders

    Background: Disability associated with mental illness is a major contributor to the global burden of disease. The present study looks at some aspects of disability associated with 7 psychiatric disorders: schizophrenia, bipolar affective disorder, anxiety disorders, depression, obsessive-compulsive disorder, dementia, and mental and behavioural disorders due to the use of alcohol. Aims: (i) To evaluate the nature and quantity of disabilities in the study groups; (ii) to compare the degree of disability with the severity of the disorder; (iii) to compare disability among various disorders; and (iv) to study the longitudinal stability of disability in the disease groups. Methods: A total of 228 patients attending the OPD, Department of Psychiatry , Assam Medical College, Dibrugarh, between July 2003 and June 2004, who were diagnosed as per ICD-10 guidelines and SCAN, were included in the study. Severity was assessed by the application of some commonly used rating scales for each specific disorder. The level of disability was assessed by using the Indian Disability Evaluation and Assessment Scale (IDEAS). Patients were followed up at 6 and 12 months. Statistical analysis was done on SPSS version 10. Results: All the 7 disorders under study are associated with significant disability; schizophrenia being maximally disabling. Disability associated with alcohol use disorder and anxiety is comparable to disability on account of OCD. Over a period of 12 months, disability due to depression, alcohol use disorder and anxiety tend to remain significant.
