    Q. Dimorphic fungi are fungi that can reproduce as either a mycelial or a yeast-like state. Generally the mycelial saprotrophic form grows at 25° C, and the yeast-like pathogenic form at 37° C. Dimorphism is extremely important in the identification of mycoses, as it makes rapid identification of many pathogenic organisms possible. Some diseases caused by dimorphic fungi are

    sporotrichosis
    blastomycosis
    histoplasmosis
    coccidioidomycosis
    paracoccidioidomycosis
    penicillosis

15.regarding ACE inhibitor which of the following is true
    1. inhibits conversion of angiotensinogen to angiotensin1
    2. t ½ of enalapril is more than lisinopril
    3. ommiting the diuretic dose helps in reducing first dose hypotension
    4. it is effective only with left ventricular systolic dysfunction

    Answer is C.....
    Harrison 15TH ED 1425

ACE INHIBITORS:- These drugs should be used with caution when the renin system is activated (e.g. by severe heart failure, prior diuretic therapy, or substantial salt restriction) to avoid profound hypotension.Renin system activation means hemodynamics is maintained by the activation of the same, which would have been otherwise compromised. Usually diuretics should be stopped 2 to 3 days before administration of an ACE inhibitor is begun and are added back later if needed.

It can however be used as combination therapy. If ACE inhibitor was not the part of combination therapy with thiazides, caution should be used as administration of such an agent to a patient who is already taking a diuretic occasionally may lead to profound hypotension.
