    Major Risk Factor Screening Technique
History of cardiac arrest or spontaneous sustained ventricular tachycardia History
Syncope Usually with or after exertion History
Family history of sudden cardiac death Or possibly with a documented gene mutation associated with high risk Family history
Spontaneous nonsustained ventricular tachycardia >3 beats at rate >120 Exercise or 24–48 hour ambulatory recording
LV thickness >30 mm Present in about 10% of patients, but many sudden deaths occur with wall thickness <30 mm Echocardiography
Abnormal blood pressure response to exercise Systolic blood pressure fall or failure to increase at peak exercise Maximal upright exercise testing

capillary leak syndrome (CLS) has been reported in recipients of
    a) Filgrastim
    b) Pegfilgrastim
    c) GM-CSF
    d) G-CSF

ANS: NONE (according to latest 11th edition of omkarnath JIPMER book)

explanation is nicely given... according to omkarnath book.. it says

All of the options given in the Question contribute in the causation of Capillary leak syndrome (CLS).

    > Capillary leak syndrome (CLS) or Systemic capillary leak syndrome, SCLS, or Clarkson’s disease is characterized by
self-reversing episodes during which the endothelial cells which line the capillaries are thought to separate for a few days, allowing
for a leakage of fluid from the circulatory system to the interstitial space, resulting in a dangerous hypotension (low blood
pressure), hemoconcentration, and hypoalbuminemia. It has been reported in recipients of filgrastim, including patients undergoing
chemotherapy and a healthy donor undergoing peripheral blood progenitor-cell mobilisation; it has also been reported
in recipients of pegfilgrastim undergoing chemotherapy. Episodes varied in severity and frequency. CLS is characterised
by: hypotension and oedema; hypoalbuminaemia; and haemoconcentration, and may be fatal unless promptly diagnosed and
    managed.

    Other Drugs causing CLS include: GM-CSF; G-CSF; IL-2; IL-4; Gemcitabine; Docetaxel; Denileukin; Aldesleukin; Clofarabine;
    Denileukin diftitox & Bevacizumab

    > Systemic capillary leak syndrome occurs even after following administration of granulocyte colony-stimulating factor (G-CSF).

    lithium stopped before 48 hrs
for post puncture headache--wrong statement wz...stylet kept inside and needle removed

ithium's direct effects cause hazardous risks in surgery. This is specifically true when haemodynamic instability occurs and renal excretion becomes impeded through interference with sodium and potassium metabolism. Therefore, lithium discontinuation is recommended. Lithium can be stopped at once because no withdrawal symptoms occur. Taking a half-life of 24–36 h into account, lithium should be discontinued 72 h before surgery
