i personally think that as u r already pursuing ur dgo which will be over soon, and u will be reinstated as medical officer in esi..with govt pay scales and perks u should continue with ur job..coz-
    1.job is permanent
2.u will be considered a specialist
    3.due perks
    4.central govt pay scales
5.already dgo -that is diploma in a good clinical branch

but since thats not the question u asked after psm pg u can get an sr ship in any of the medical colleges-its not very easy though;u can also land up witha WHO job-thats a possibility but not something all pg psm get;u could do mph etc..

but i would suggest u to weigh the pros of a permanent clinical job with good scales as compared to a 3 yrs more of student life that too semi clinical with no surety of a job.

    Hi....

Apply for Leave - Loss of Pay (UEL without Medical Certificate) and do your MD SPM.....

YOu can join in ESI and will be posted in District or Zonal level Cadre (after giving an apology and loosing one or two increments for your LONG LEAVE)

There is no need to quit from service now

as far as i have inquired about the esi rules no ucannot be eligible for leave and would have to join or the seat stands forfeited.
the leave applies after a fixed term of 3 to 5 yrs of esi service.

YOu can avail loss of pay any time..... Only for study leave or medical leave there are criteria like regularisation and probation etc.... Please verify with some central govt officials...... (I am conversant with State Govt Rules)

regarding job prospects in community medicine

    dear doctor.
no doubt job prospects in community medicine r gud. but i think u might hv better opted for microbiology or biochemistry which are the upcoming branches of tommorrow, there r more job opprtunities even in WHO/ International organisation if one is a microbiologist. so is the case with biochemistry. there are well reputed international labs in india and abroad funded by WHO which pay highly.

as far as my ranking is concerned...
MD MICRO> BIOCHEMISTRY > COMM MEDICINE
