    50 odd years old patient with persistently high BP. Previous Lisinopril caused angio-oedema, Nefidipine caused severe flushing and the patient stopped taking all medication as he was fed up. Now coming with a pulse of 100 and BP 158/92. hypertensive retinopathy on fundoscopy. What is the best Mgx?

    for macroprolactmeamia,I put surgery.
    forhand warts I did not put cryo because it was so plenty all over.
    for syrotonin syn. I put Dantrolene.
    ch.pancreatitis for the steatore.
    patient with the high bp it was a slide ECG with LVH so my answer losartan.

    51 year old with gastritis on endoscopy,+ hpylori,given treatment.reviwed after 8 weeks he was asymtomatic (2 paper)
    1 keep in maintenance therapy
    2 reasure and discharge.
    3 repeat endoscopy.
    4 breathing test for h-pyl.
    5 serelogy test for h.pylori.

    my answer was dischare.


    the same hisory in another 3 paper but the finding was bleeding du and treatment given .after 8 weeks he was asypomatic. the same options wewe given.

    I have a questino from onexamination .com on April 2005 the comment said ((IF THE PATIENT IS H.PYLORI +VE GASTRITIS WITH NO ULCER, HE SHOULD RECEIVES A 7 DAY COURSE OF STANDERD ERADICATION TREATMENT AND BREATHING TEST IS UNNECESSARY IF ASYMTOMATIC , UNLESS HE HAS BLEEDING OR PERFORATION OF A PEPTIC ULCER OR THERE ARE RISK FACTORS e.g.ANTI-COAGULATION THERAPY OR NSAID THERAPY .IN SUCH CIRCUMSTANCES THE H.PYLORY BREATHING TEST IS DEFERRED FOR 5 WEEKS AFTER THE CONCLUSION OF ERADICATION THERAPY)).

    I am pretty sure the answer was nasopharyngeal CA not glomus jugulare because there was rt sided conductive recent onsent deafness that could only be possible with Naso pharyngeal CA.

    Question asking for site of lesion with EMG findings 2 muscles with fasciculations & fibrillation, others showed sparse fibrillation?

    there was q about an elderly operated for #neck of the femer and after 10 days she developed sudden chest pain sob hypotensio 60/30 sweating,cyaosed the mamagement;
    1 intubation with positive presure.
    2 ?analgesia
    3 ?continueos oxygen

    the q about the thyrotoxicosis on carbimazole pation develops general body rash after URTI there wewe blood investigations with it .the CBC showed lymphcytosis without any neutropenia the options after carbimazole stooped
    1 RAI
    2 propranolol
    3 thyroidectomy
    4 propylthiouracil
    5..........

    I put pripylthiouracil the rash propaply from the URTI the surgery and the RAI might be unsuilable for acut attach and to stop the treatment completely because of the rash only may be not right
    but still i am not sure about the right answer

    I have asked one of the endocrine consultants and he thinks that you should try propile thio uracil and get the pt settled b4 radioactive idodine because sometime RAI can cause destruction and release of stored hormones causing grave detioration of thyrotoxicosis if given in acute phsase

    I gave RAI though.......................... :(

    Guys I have done the diabetic clinic first time in my life with my consultant today.Interestingly same scenario came with almost same kind of random glucose levels.28 weeks pregnant.glucose 7.4--in exam.7.2,glycosuria ++ proteinuria +.....I asked the consultant.He asked for fasting BLOOD GLUCOSE.I put the same qs. to him......he said if the qs.aske was---what would U do/what is the next step then the answer was-------fasting sugars----but -----if the qs. was best investigation then the answer was------OGTT-----------REGARDS.
